Im making a site for a heart surgeon. I intend to add forms pdf that patients can fill in before visit. What is the best way to do this? Is there a way that they can fill in the forms online … or do they have too have correct software their end?
Have a look at Formsnap. Not a PDF, but seems to have the functionality you’re after, and can save to Google Sheets and/or MySQL.
Disclaimer: I’m not a legal expert, so please consider this as general information and not legal advice. During my extensive tenure as the lead web designer for a large integrated health system, I had the opportunity to explore various solutions for RapidWeaver, including my personal favorite, MachForm, known for its secure features. However, it’s crucial to note that none of these options met the stringent HIPAA standards set by our legal team.
Ultimately, we had to opt for a .NET solution hosted on our highly secure network to ensure compliance with HIPAA regulations. Violations of HIPAA can result in significant financial repercussions, so it’s essential to prioritize security and compliance when dealing with sensitive medical data.
Thankyou a million times for taking the time to give me this invaluble information!
paperform.co which is another 3rd party form maker has the option to NOT store responses on their server - I think that is partly for HIPAA compliance.
Brilliant Thank you, I’ll take a look
It should be noted that Machform is not a RapidWeaver solution.
Jotforms, Formstack and Kiteworks all claim HIPAA compliance. Again these are not RW solutions but should work perfectly well. Will likely require a subscription.
